MUSEUM OF CONTEMPORARY ART / JACKSONVILLEwww.MOCAjacksonville.org CALL FOR NOMINATIONS MOCA Jacksonville’s 2010Memphis Wood Excellence in Teaching AwardWinner receives: $1,000 cash award, free museum tour for class, and award plaque Memphis WoodKnown as Jacksonville’s First Lady of Art, Memphis Wood made an astonishing contribution to art and art education in Jacksonville that is still felt today. As a one-woman force in building an appreciation for the arts, Memphis taught and influenced many of Jacksonville’s most successful artists. Until her death in 1989, Memphis had worked for over 60 years in Jacksonville as an artist in many media—painting, pottery, sculpture, jewelry design, printmaking, and imaginative fabric collages. Memphis Wood’s connection to MOCA Jacksonville was long and personal, both as one of its founding trustees and as a longtime studio art teacher. MOCA believes that visual art is a vital component of P-12 education. It is important not only as a distinct discipline, but also as a means of promoting student learning through integrating into other academic disciplines. Building on Memphis Wood’s devotion to art, art education, her community leadership and her conviction that art should be accessible to all, MOCA is proud to offer this annual award in her honor. Eligibility The competition is open to all P-12 educators in Baker, Clay, Duval, Nassau, and St. Johns counties.  Nomination GuidelinesThe nominee or a second party may make nominationsAnswer all questions and submit all materials as 8.5 x 11 hard copies inserted in a plain, one inch 3-ring binder as a portfolio entry (please submit print media only; no video, etc.) Selection CommitteeThe selection committee will consist of community leaders involved with education and/or the arts. NOMINATIONS DEADLINE: Friday, January 29, 2010 Please submit your nomination portfolios to:Memphis Wood Excellence in TeachingMuseum of Contemporary Art Jacksonville333 North Laura StreetJacksonville, FL 32202 Questions: Contact MOCA’s education department @ (904) 366-6911 x 207 or e-mail agalloway@MOCAjacksonville.org    MUSEUM OF CONTEMPORARY ART / JACKSONVILLEwww.MOCAjacksonville.org MOCA Jacksonville’s 2010 Memphis Wood Excellence in Teaching Award Nominee or second parties may make nominations.Nominations are open to all P-12 First Coast teachers. APPLICATION Nominee Name: ———————————————————————————————————————————————————————————————————— School/District: ————————————————————————————————————————————Address: ————————————————————————————————————————————————————————————————————————————————————————————————————————————————————————————————————— City: ——————————————————————————————————————————————————————————————————————— State: Florida Zip: ————————————————————————————Phone: ———————————————————————————————————————————————————————————————————— E-mail: —————————————————————————————————————————————————————————Subject Taught: —————————————————————————————————————————————————————————————————————————————————————————————————————————————————————————————— Nominator  Same as AboveOrName: ———————————————————————————————————————————————————————————————————— Organization: ————————————————————————————————————————————————Address: ————————————————————————————————————————————————————————————————————————————————————————————————————————————————————————————————————— City: ——————————————————————————————————————————————————————————————————————— State: Florida Zip: ————————————————————————————Phone: ———————————————————————————————————————————————————————————————————— E-mail: —————————————————————————————————————————————————————————Relationship to nominee: —————————————————————————————————————————————————————————————————————————————————————————————————————————————————    MUSEUM OF CONTEMPORARY ART / JACKSONVILLEwww.MOCAjacksonville.org  Please describe you/the nominee’s teaching excellence by responding to each of the following questions.
